Consent to Verification of Credit and Other Information:
I am making this Application voluntarily for the purpose of obtaining GWM's approval to rent a property in the community listed on page 1 of this application. I hereby authorize and consent to allow GoldenWest Management, Inc, Owner, and their respective employees and agents (collectively "GWM PARTIES"), to obtain and verify the credit and other information provided by me in this Application through credit reporting agencies, tenant screening service companies, banks (including electronic funds verification), employers and other persons or entities with information relating to the Application. I also authorize the "GWM Parties" to provide information contained in the Application to various local, state and/or federal government agencies, including without limitation, various law enforcement agencies. I understand that if I lease this property, the GWM Parties shall have a continuing right to review my credit information, payment history, occupancy history and other information in this Application for purposes related to my Lease and/or for account review both during and after the term of my lease.

I hereby release and hold harmless GoldenWest Management, Inc, and all of their respective officers, brokers, employees and agents, from any and all liability, legal proceedings and costs, including attorney's fees, arising out of the verification and/or use of the information contained in this Application, including the release of such information to other parties in order facilitate the verification of information or to track down Tenants in the event of legal action.

I warrant that, to the best of my knowledge, all of the information provided in this Application (including but not limited to the statement of my financial condition) is true, accurate complete and correct as of the date of this Application. If any information provided by me is determined to be false, such false statements will be grounds for disapproval of my Application. If any information provided by me is determined to be false, such false statements will be grounds for disapproval of my Application or termination of my Lease with Owner. I agree to notify GWM if any of the information provided in this Application changes during the Application process or during my tenancy. I also understand that GWM will remain this Application, along with any other information provided by me, whether or not this Application is approved.

Applicant understands and agrees; (i) this is an application to rent only and does not guarantee that applicant will be offered the Premises: and (ii) Landlord or Manager or Agent may accept more than one application for the Premises and, using their sole discretion, will select the best qualified applicant.

If application is not fully completed, or received without the screening fee: (i) the application will not be processed, and (ii) the application and any screening fee WILL NOT be returned.

Screening Fee
Applicant agrees to pay a NONREFUNDABLE screening fee of $20.00, applied as follows:
